ABSTRACT:
Background: Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder that adversely affects reproductive outcomes in women undergoing ICSI. Premature LH surges during controlled ovarian stimulation (COS) can compromise oocyte yield, particularly in high-responders. Objective: To compare follicular response and hormonal profiles in PCOS women undergoing ICSI following suppression with either Medroxyprogesterone Acetate (MPA) or GnRH antagonist protocols. Patients and Methods: This study enrolled 108 infertile women with PCOS undergoing ICSI at the Al-Nahrain University Center between December 2023 and May 2025. 59 participants received GnRH antagonist protocols, while 49 were treated with MPA. Parameters assessed included LH levels, estradiol, oocyte maturity, and ovarian sensitivity index. Results: LH levels on day 2, day 6, and trigger day showed no significant differences between groups (p=0.62, 0.26, 0.14). The MPA group had a slightly higher, but non-significant, number of retrieved oocytes (p=0.83). Gonadotropin dose, duration of stimulation, ovarian responsiveness, and mature oocyte (MII) yield were comparable between groups (p>0.05). Oocyte maturity did not differ significantly. Conclusion: Medroxyprogesterone acetate is an effective, safe, and cost-efficient alternative to GnRH antagonists for preventing premature LH surges in PCOS women undergoing ICSI, without compromising ovarian response, oocyte maturity, or stimulation outcomes.
