ABSTRACT:
Captopril, an antihypertensive drug from the ACE inhibitor class, is widely used as a first-line treatment for hypertension. In clinical practice, it is often combined with herbal medicines as either an alternative or complementary therapy. Centella asiatica leaves are commonly used as an adjunct in hypertension treatment, typically in the form of an infusion. These leaves contain various secondary metabolites known for their blood pressure-lowering effects. However, concurrent use with captopril may impact the drug's bioavailability. This study aimed to investigate the effect of C.asiatica leaves infusion on the pharmacokinetics of captopril in rats (Rattus norvegicus). The study employed an observational design with a time-series method. The two groups received different treatments: the first group was given captopril alone, while the second group received a combination of captopril (0.45mg/200g.BW) and C. asiatica leaves infusion (3.25ml/200gBW). Plasma captopril levels were analyzed using UV-VIS spectrophotometry at 205 nm, with measurements taken at 0, 1/2, 1, 2, 4, 6, 12, and 24hours. The results showed that the combination of captopril and Centella asiatica leaves infusion significantly altered (p<0.05) the pharmacokinetic profile of captopril. Specifically, the combination prolonged the half-life (t1/2) from 2-3hours to 10hours, delayed Tmax from 1 hour to 2hours, and reduced both Cmax and the elimination rate constant (Ke1) from 0.207hour-1 to 0.069 hour-1. These findings indicate that Centella asiatica infusion modifies the pharmacokinetic characteristics of captopril.
