ABSTRACT:
Diabetic foot ulcers present a major clinical and economic challenge, contributing significantly to global morbidity and mortality. This meta-analysis research evaluates the efficacy of chitosan and Aloe vera dressings in the management of diabetic foot ulcers. Published randomized controlled trials, case-control studies, and cohort studies from January 2000 to December 2024 were taken from PubMed, Cochrane Library, EMBASE, and CINAHL. This protocol was registered with PROSPERO (CRD42024587596). Out of 1,643 articles (1,300 on chitosan and 343 on Aloe vera), six studies met the inclusion criteria. Only two Aloe vera studies qualified—one on ulcer closure and one on analgesic effect—providing insufficient data for conclusions. Four studies on chitosan were included. One RCT (n = 262) showed reduced healing complications with chitosan dressings (OR = 0.81; 95% CI: 0.65–1.01; I² = 35.2%). Combination therapies (n = 186) also showed benefits, though with high heterogeneity (I² = 81.9%). Chitosan-based dressings demonstrated promising outcomes in reducing diabetic foot ulcers complications, though moderate to high heterogeneity suggests variability among studies. Larger, high-quality studies are required to confirm the safety and efficacy of treatment strategies. Evidence for Aloe vera remains inconclusive.
